Instructions for Opening Your Account.

Thank you for inquiring about Mutual Bank for your current investment needs. Mutual Bank is a Chicago area Community Bank and not just an internet bank. We currently do not open accounts through our web-site but we offer an easy alternative.

Attached is a Consumer Application which will gather all of the information we need to establish you as a customer of Mutual Bank. One Consumer Application is required per individual, so complete one form for each signer who will be on any account, not one for each account. For example, if you are opening a joint account with one other co-owner, you would fill out two forms, one for each of you.

If you would like to name a beneficiary who would receive the funds easily in the case of your death, you may do so by filling in their name and address under “Revocable Trust with Beneficiary in the “General Account Information” section. We would not need Consumer Applications on any beneficiaries listed. If there are several different ownerships when opening more than one account, it would be better for you to explain that in a letter of direction and leave the top section of the Consumer Application titled “General Account Information” blank.

In addition to the Consumer Application, we also need your signature on a W-9 form to verify your social security or ITIN number. This will attest to the fact that you are not subject to back-up withholding on your interest. We have provided a link to the IRS web-site at http://www.irs.gov/pub/irs-pdf/fw9.pdf. Please print one of these forms for each signer. You may fill in your name, address and social security number on-line, then print, sign and date the document. Send these along with your application form(s).

The USA PATRIOT Act requires Mutual Bank to verify your identity by authenticating your name, address, date of birth, Social Security or ITIN number and a second piece of identification. For this purpose we require that you return clear copies of your driver’s license / state identification card and one other piece of identification (library card, voter registration card, insurance card, etc.) with the correct name and address along with the above documentation.

Here is a list of what you will be sending to us to establish your account:

Note: Any information missing on the Consumer Application or any required documents not provided could result in the account not being opened and the documents returned.

  1. The completed and signed Consumer Application(s).
  2. The completed and signed W-9 form(s).
  3. A copy of the driver’s license for each signer (front & back).
  4. Copy of 2nd ID with proof of address
  5. Provide mailing address if different from address listed on Consumer Application- P.O Boxes are for mailing purposes only.
  6. For Checking or Savings ONLY - Provide 3rd form of ID - choose from list provided on the Consumer Application.
  7. Your letter of direction
  8. Your check made payable to Mutual Bank for your deposit.
  9. If you would like interest transferred to another bank account, please include a copy of a voided check for that checking account.
  10. If the account will be owned by a formal trust, copies of the front page, designation of trustee pages and signature pages must accompany your application.
